This Privacy Policy sets forth the general privacy practices of the Northwest Minnesota Foundation as pertain to the MyPath web-based application (the “App”).

The Northwest Minnesota Foundation is committed to respecting your privacy rights when using the App. To protect your personally-identifiable information, the Northwest Minnesota Foundation has adopted the guidelines outlined below.

 

The App is available only to persons resident in the United States.

 

Types of Information Collected

When you use the App, we may ask you to register and/or provide information that personally identifies you for purposes of using the App and putting you in touch with resources, agencies and referrals. Such personal information may include, but is not limited to, your name, age, phone number, street address, and e-mail address, as well as information concerning requested services and your living and housing situation. On any page that collects personal information, we will specifically describe what information is required in order to provide you with the resource, agency, or referral that we believe best fits your needs in accordance with the information you give to the App, as well as to respond to your inquiry or comment.

 

Email is not a secure communications medium. We will not request you to provide personal information via email.

 

Use of Personal Information

We believe in protecting your privacy and will not share personally-identifiable information with third parties outside of Northwest Minnesota Foundation except as described in this Privacy Policy. We use personally-identifiable information about you to identify you when you use the App, to improve our efforts to make the App better known to people who might benefit from it, to statistically analyze App usage, and to put you in touch with resources, government, nonprofit and other agencies, and referrals, as well as with governmental authorities and law enforcement, if we believe that you are in imminent danger. We may also use your personally-identifiable information, such as your e-mail address or mailing address, to contact you regarding resources, agencies and services that may be of interest to you, to respond to an inquiry from you regarding the information that we have collected, or respond to your comments and suggestions.

 

Northwest Minnesota Foundation will not sell your personally-identifiable information to third parties. Northwest Minnesota Foundation also may disclose aggregated user statistics in order to describe the App to current and potential partners, donors, governmental agencies, and other third parties, as well as their respective affiliates, and for other lawful purposes.

 

We reserve the right to disclose personally-identifiable information when needed to comply with applicable law or any lawful request or legal process, cooperate with investigations (including, by governmental authorities and law enforcement) of purported unlawful activities, to identify persons violating the law or in imminent danger, to protect the property or rights of Northwest Minnesota Foundation, third parties, the public or others.

 

Cookie Policy

 

What are Cookies?

A cookie is an element of data that a website or application can send to your browser, which may then be stored on your system. This element of data is a piece of text, not a program. The App can only access the information from a cookie sent by the App. We cannot access other cookies sent by other websites or applications or the information contained therein. Additionally, we cannot learn your e-mail address or any other information about you through the use of a cookie. The only way we would learn such information is if you specifically and voluntarily submit that information to us, for example, through a registration system, by entering a sweepstakes or promotion, etc.

 

Why do we use cookies?

Northwest Minnesota Foundation uses cookies to track usage of the App and further personalize your experience when you are using the App. By tracking usage, we can best determine what features of the App best serve the users.

 

What benefits do I receive from cookies?

Overall, the use of cookies helps to give you a more personalized experience with the App. The information is used to keep our App and the services offered through the App fresh and relevant to you, the user. Cookies also allow you to avoid retyping your e-mail address and password each time you access the App (if applicable).

 

What happens with the information collected in a cookie?

The information stored in a cookie is used to identify you to our server, to allow us to personalize your experience, and to serve content and communications that may be of interest to you.

 

May I decline to accept a cookie?

BY USING THE APP, YOU CONSENT TO THE PLACEMENT AND USE OF COOKIES IN ACCORDANCE WITH THIS PRIVACY POLICY. You may decline to accept cookies by selecting an option on your browser to reject cookies. However, some portions of the App may not be usable or optimized without cookies, or may require registration in order to access those areas. In such cases, we ask that you register and provide us with basic information on our registration page, or sign in if previously registered. Cookies enable our server to know that you are a registered user and will eliminate the need for you to log in each time you access the App. In addition, our registration system may require that you accept the cookies in order to access that portion of the App.

 

IP Addresses

Northwest Minnesota Foundation does log IP addresses, or the location of your computer on the Internet, for systems administration and troubleshooting purposes and to determine App usage and for other purposes permitted pursuant to this Privacy Policy. For purposes of this Privacy Policy, IP addresses are not considered part of your personally-identifiable information, regardless of whether IP addresses, or the location of your computer on the Internet, may potentially be personally identifiable.

 

Party Collecting the Information

This Privacy Policy only addresses the use and dissemination of information that Northwest Minnesota Foundation collects from you. To the extent that you disclose any information to other parties, whether through, for example, contacting an agency referred to you through the App, or the like, whether directly or through the Internet, different policies may apply and Northwest Minnesota Foundation shall not be responsible for the use or dissemination of such information whatsoever. In addition to, and without limitation of the foregoing, with respect to any information (including any personally-identifiable information) you provide or disclose on the App, which is made available to any third party, such information will no longer be private or confidential and Northwest Minnesota Foundation shall not be responsible for the use or dissemination of such information whatsoever and such information shall no longer constitute personally-identifiable information subject to this Privacy Policy.

 

Links to Other Materials

Third-party websites (including links to government and nonprofit agencies) linked to and from the App are not under the control of Northwest Minnesota Foundation. We shall have no responsibilities or liabilities whatsoever for the content or privacy practices of any such linked website or any link or linking program at any time. We do not endorse companies (or related products or services) to or from which the App is linked merely by reason of the existence of any such link. If you decide to access any of the third party websites linked via the App, you do so entirely at your own risk.

 

Children’s Privacy

We care about the safety of children and their use of the Internet. Therefore, in accordance with the Children’s Online Privacy Protection Act of 1998, we will never knowingly request or solicit personally identifiable information from anyone under the age of 13 without verifiable parental consent. In the event that we receive actual knowledge that we have collected such personally-identifiable information without the requisite and verifiable parental consent, we will delete that information from our database as quickly as is practical.

 

Opting Out and Requesting Deletion of Personal Information

You may choose to “opt-out” and request your personal information be deleted at any time. The App, however, does require registration, and registration cannot be completed without you providing some personally identifiable information. If you choose not to register or provide certain information, or choose to delete your account, the App or certain functionalities may not be accessible to you.

 

California Residents

Reviewing, Changing and Deleting Your Personal Information

If you have made a user account, you may access and maintain all of the personally-identifiable information that we collect from you online by using the procedure that follows. We use this procedure to better safeguard your personally-identifiable information. To protect your privacy and security, we will also take reasonable steps to verify your identity before granting access or making corrections.

 

You may also, at any time, choose to change or delete your profile from the App using this procedure. You should know that as a result of deregistration, the App may no longer be accessible to you.

Northwest Minnesota Foundation maintains reasonable industry standard safeguards to protect the security, integrity, and privacy of the personally-identifiable information that you provide to Northwest Minnesota Foundation, however no transmission or storage system, and no transmissions of information, involving the Internet are completely secure, and therefore Northwest Minnesota Foundation provides no warranty or guaranty as to the security, integrity, or privacy of the personally-identifiable information that you provide to us, and you do so at your own risk.

 

Privacy Policy Changes

Northwest Minnesota Foundation may change this Privacy Policy at any time and from time to time without notice, in Northwest Minnesota Foundation’s sole discretion. Northwest Minnesota Foundation encourages you to frequently check this page for any changes to this Privacy Policy. Your continued use of the App after any change in this Privacy Policy will constitute your acceptance of any such change.

Who we share your data with?

 

How long we retain your data?

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

 

What rights you have over your data?

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us.
